Showing or Hiding Consent Forms

You may want to show or hide consent forms depending on the answer to a question. For example, when sending contracts, if the patient chose braces you probably don't want to include the Aligner Consent Form. Another option may be to show the Retainers for Life form if they chose that option. You can also let the patient or responsible party choose their language.

Start by adding one or more questions to the Contract Questionnaire. They should Question type: Multiple-Choice - Single Answer

Once the question is added, check that all consent forms are attached

Add Rules

Click on the three dots (○○○) and select Rules

Add New Rule - you can show or hide a consent form

If you use show then that form will only display if the question is answered as selected.Be sure to show consent forms at the bottom of the questionnaire in Settings (under the three dots (○○○))
