Updating Consent Forms
From time to time, you may need to update your consent forms.
Go to My Form Templates
Then select the Consent Form you wish to update.

It's best to ALWAYS make a backup - click on the ○○○ and select Duplicate

There are two types of consent forms: PDF and Rich Text
You may ne ed to upload an updated version of your existing PDF and then add any text boxes and staff prefills, mapped labels, signatures, and initials. Any questionnaires that the form is linked to will then have the updated form.

For existing PDFs you can also change text boxes and staff prefills, mapped labels, signatures, and initials.

Rich Text
For Rich Text, you can edit the actual text. Just open the form and type in the Form Content box. For more detailed instructions see Create Consent Forms
⇒ Please note if you are using Consult Manager, the Financial Contract forms contain embedded tables. Their formatting may be corrupted if edited incorrectly!


Please note than you cannot simply copy and paste a mapped label, initials, or any inserted field. You must add a new one.
To add a row to a table, click on the row above where you want the new row and select insert row below
