Mapped Labels for Contracts

Your Gaidge Consult Manager contracts use mapped labels to insert patient, responsible party and signed contract data. Additional data can be added.

Updating your financial agreement form templates

Begin by going to Forms on your left-hand menu, then select My Form Templates. 


⇒ Please note the Financial Contract forms contain tables which may be corrupted if you do not know what you are doing!
 

Scroll, if needed, to your Consent Forms. The Consult Manager contracts generally reside in a folder called GCM Contracts or Financial Arrangements. Click to open this folder and select the agreement you want to edit. Scroll to the place you want to add information and type a description, e.g. Treatment Notes, if desired. 

Next click on Insert Field on the black editor bar, then choose Mapped Label. This will insert a mapped label where your cursor is blinking on the page.

Click on Profile Mapping, then scroll until you find Tx Notes (this will be the last item for most practices), select it and click OK

Please note that many of these options are standard forms data, not necessarily related to contracts. If your new patient form does not map these to the patient profile in forms, the data is not available.
