E-Signature Question Type

An e-signature question type can be used anywhere in your forms. They add a signature field to your form for patients to sign.

Before we look at what you can do with the e-signature question type, it’s important to note that consent forms have their own e-signature box at the end of the document by default so you do not need to add one. 

After entering your text, there are two additional options:

• Is Required - patient must sign in order to submit form
• Display Date field on printed forms - when printing, an empty date field will be printed beside the signature line

What your patient sees:

Under Question Options, select For Office Use Only to add a signature for staff use when the form is submitted.
