How to Send a Patient Consent Forms Only

If you want to send a single consent form to a client, refer to this article. If you want to send more than one consent form please keep reading.

In this scenario, consent forms need to be linked to a Forms Package. To create a forms package that contains only consent forms (no questionnaire), follow the steps below:

1. Navigate to Templates.
2. Click on the box + Create New in the Questionnaires section, give the form a name, and click Continue.

You now have an empty forms package that doesn’t contain a questionnaire. We suggest you add a field with instructions to it. To do this, click on the Add New Question button, select the question format Section Title / Note and inside the Section Text (Optional) box, type whatever instructions you want to give your patients.

You may now click on the  Consent Forms button and select any forms you would like your patients to sign (select them in the order you want them to appear on the form). Now click on Preview to see what the form will look like to your clients:

If everything looks good, click on the menu button,  Send to Client, or click on More > Get URL if you want to place the link to the forms on your website.
