How to Send a Consent Form to a Client

To send only one consent form to a client, without the need to create a forms package, follow the steps below:

1. Navigate to Templates and open up the consent form you'd like to send.
2. Click on the Send to Client link in the left menu.
3. Type the patient's name and email.
4. Click on Send Form.

This will create a forms package on the fly, containing a single consent form.
