How to Send a Form to a Client

To send a form to a client, follow the steps below:

  1. Log in to Gaidge Forms.
  2. On the dashboard, click on the big blue button named Send Form to Client.
  3. Select the appropriate form from the dropdown list.
  4. Type the patient’s name and email or click the Email drop down, select SMS and enter mobile number.
  5. Add a note to your patient, if desired. This note will be displayed above the first question of the form.
  6. Click on Send Form.

That’s it. You’ll be notified when your patient completes the form.

If you need to see the form you just sent, click on Client Forms and search for the patient name.

You can also send forms by adding forms to your website, or by placing a link to forms on an iPad/tablet device.
