Uploading Existing Forms- Need to Know

How to Upload Your Existing Forms

Need to use forms you already have? The Forms team can help convert your existing documents into digital forms, making it easier to manage your practice’s paperwork.

This guide explains how to prepare your files, upload them to your account, and avoid common delays.

Prepare Your Forms

Before uploading your documents, make sure they meet the following requirements:

  • Use a supported file format. Submit your forms as Microsoft Word documents or PDFs with selectable, copyable text.
  • Avoid scanned documents. Image-only scans cannot be accepted because the text cannot be copied.
  • Include one form per file. Each document should contain only one form.
  • Review your content. Forms are converted exactly as submitted, so make sure all information is accurate and up to date.
  • Remove credit card information fields. Forms requesting raw credit card details cannot be converted.
  • Ensure there is no PHI in the form. Information cannot be removed from forms, so make sure all fillable fields are empty.

Helpful tip: You can submit up to 10 forms per day.

Upload Your Existing Form

Once your files are ready, upload them to Forms > My Forms Templates > Consent Forms > Upload Existing Form.

Allow Time for Processing

Most form uploads are completed within 2–4 business days, although turnaround times may vary.

Our Forms team converts your forms using the files you provide. If you need to make changes after submitting a request, send an updated document so the correct version can be used.
